Enclosed is a packet of the BEATRICE DAVIS LEADERSHIP SEMINAR materials to be used in applying for the 2001 Scholarships. Read the entire packet of information.

So that your application can be considered, it is imperative that you comply with the directions for applying. Funding for this special Scholarship was made available through a bequest from the estate of Miss Beatrice Davis, a past Louisiana State President and Southeast regional Director.

3.Ten (10) copies of APPLICATION FORM

4.Chapter President RECOMMENDATION FORM (Original only)

5.Professional Referent RECOMMENDATION FORM (Original only)

6.Verification of 2000-2001 DUES PAYMENT from Treasurer

Note that it is YOUR RESPONSIBILITY to send a recommendation form and a self-addressed stamped envelope to each referent and to request that the completed form be returned directly to you. These recommendation forms must be mailed with your completed application form to Virginia Crossno, POSTMARKED NO LATER THAN DECEMBER 11, 2000.It is recommended that the packed be sent by CERTIFIED MAIL.

Guidelines for Application

CRITERIA FOR SELECTION

1.A minimum of three full years as a member in good standing of Epsilon State.

2.Evidence of:

a.Leadership potential as demonstrated by holding chapter office or involvement in committee work of Delta Kamma gamma

b.Professional involvement

3.Attendance at an Epsilon State District Meeting or State Convention

4.Statewide representation

5.The following persons are not eligible for participation:

a.Present or past elected state officers

b.Former international Golden Gift Leadership/Management Seminar participants

DIRECTIONS FOR APPLYING FOR SEMINAR

1.Applicants must submit their applications on forms which will be furnished on request from the Chairman of the Leadership Committee, Virginia Crossno, 225 Jefferson Street, Natchitoches, Louisiana 71457. Additions to the form will not be accepted.

2.The original and ten (10) copies of the application must be sent to the Chairman of the Leadership Development Committee.

3.Applications and letters of recommendations must be POSTMARKED by December 11, 2000.

4.The application must be signed by the applicant’s chapter treasurer to verify payment of current dues.

5.The applicant must attach a small recent photo to the original application form 

6.Two letters of recommendation must be sent with the completed application form to the Leadership Development Committee Chairman:

a.Applicant’s Chapter President (or the immediate past president if the president is applying).

b.Professional person who is qualified to speak specifically of the applicant’s leadership potential.

7.Include a self-addressed stamped envelope for each referent.

8.Mail the completed application and the two letters of recommendation by Certified Mail POSTMARKED NO LATER THAN DECEMBER 11, 2000 to:

GENERAL CONDITIONS OF ACCEPTANCE

1.The successful candidates will be notified of their selection no later than March 1.

2.Within one week after notification, the successful candidate must officially notify the State Leadership Development Committee Chairman of her acceptance or rejection of the award.
